Why do I need a Beneficiary Designee?

As a participant in any of the following plans, you should provide a written designation of your beneficiary on a form provided by the Benefit Office. This designation will allow the specific benefit funds to determine who would be eligible for any applicable death benefits if you were to pass away. You must complete a form for each fund even if you are naming the same person.
